Patient Care Navigators

Click here to find out how our Patient Care Navigators can help you.

Our patient care navigators provide an important link for patients with the practice and are your initial contact point for all general enquiries. In order to do their job well and direct you to the best possible care they will ask for some information about the problem you are trying to solve. They have been asked to do this by your GP. For more information click here.

They can provide basic information on services and results and direct you to the right person depending on your health issue or query. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

Our patient care navigators are experienced in their role and specifically trained in medical terminology. They can also facilitate referrals from your doctor to hospitals and other healthcare professionals.

They work with your GP and on your behalf to make sure you are referred to the right place for your medical condition.
