A new, supportive way to manage your diabetes
We are pleased to offer face-to-face group clinics for eligible people with Type 2 diabetes. These friendly and supportive sessions provide an opportunity to spend more time with your diabetes nurse, learn from others’ experiences, and connect with people who understand the challenges of living with Type 2 diabetes. Research shows that 90% of patients prefer this approach, and patient-led group learning has been proven to significantly improve diabetes self-management.
Our diabetic nurse has recommended a group clinic to you based on your recent results.
You can join in as much or as little as you like — sharing is always optional.

How group clinics work
Before attending a group clinic, we want you to understand more about what will happen.
You will be attending a face-to-face group clinic.

At the start of your group clinic, you will consent to participate. You will be asked to sign a copy of our Group Clinic Agreement before the session.

Please arrive 5 minutes early. Your facilitator will welcome you and check your details.
