To register for these services we will require two forms of ID, presented in person at the surgery or emailed to D-ICB.sidvalleymedicalrecords@nhs.net - one of which must be photographic such as passport or driving licence and the other proof of address. We are unable to offer this service to anyone under 16 years of age.
