Age 40 - 74 years old? Unless you have already been diagnosed with one of these diseases the NHS in England will invite you for a Health Check every 5 years.

The check will assess your risk of some of the biggest causes of early death in England such as heart attack and stroke.

These conditions can often be prevented, even if you have a family history of them.

It only takes 20-30 minutes and involves a few questions and measurements, including a simple blood test to check your cholesterol.

After the check, you will receive free personalised advice to help you stay healthy.

To manage your medication and appointments online, sign up for our Online Services : 

You are now able to order repeat prescriptions for any medications you take regularly and look at your medication and allergies record online by clicking the link to Online Services from the practice website.  You can also book or cancel appointments.

To register for these services simply visit the practice with 2 forms ID, one of which must be photographic ,i.e. a passport or driving licence. The other must be an official document, i.e. a bank statement or utility bill containing your current address. This service is not available to anyone under the age of 16 years.  We are unable to allow patients to register other members of their family. Registration needs to be done in person, for facial verification purposes.

You will be given login details, so you will need to think of a password which is unique to you. 

Using the online facilities is your choice and if you wish at any time to withdraw, the practice staff will continue to treat you in the same way as before.  The decision will not affect the quality of your care.

 The practice has the right to remove online access to services for anyone that doesn't use them responsibly.
