In order to protect our shielded patients during the COVID 19 pandemic Blackmore Health Centre is closed for routine appointments until further notice.
Options for Prescription Requests normally handed in at the Blackmore Branch Surgery:
- Hand your prescription request slip directly to your usual Pharmacy (by yourself or your representative).
- Drop your prescription request slip into our main surgery letter box at Beacon Medical Centre entrance (by yourself or your representative).
- E mail request to sidvalleyprescriptions@nhs.net (including your name, address, date of birth and items required).
