In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. If you would like to request to view your medical records, please put this writing/email our Medical Records Department at the surgery, d-icb.sidvalleymedicalrecords@nhs.net. Please include a reason for your request, so that these may be processed appropriately. Requests may be subject to an administration charge, which you will be informed ahead of any work being processed. No information will be released without the patient consent unless we are legally obliged to do so.
